Parent Permission

LEGAL MEDIA RELEASE & INDEMNIFICATION:

By selecting "YES" below, I hereby grant Second Presbyterian Church, Louisville, KY, the irrevocable right and permission to photograph, record, and use my child’s likeness, voice, and participation in church activities for any lawful purpose, including digital and print media. I waive any right to inspect the finished product or receive financial compensation. I hereby release, indemnify, and hold harmless Second Presbyterian Church, its staff, and its volunteer leadership from any and all claims, damages, or liabilities arising out of the use of these media materials.

ADMINISTRATIVE DURATION POLICY:

By completing this registration, my media preference will remain securely on file as the church's active policy until I notify the church office otherwise. If I have previously opted in and now wish to update my preferences or opt out, I understand I must contact the church office directly at 502-895-3483 to submit a formal change request.

I also give my permission for these photos and videos to be posted on Second Presbyterian Church's website and social media accounts, as well as in Second Presbyterian Church's email blasts and printed publications. I understand and agree to the legal release language above, and note that Second Presbyterian Church will never include my child(ren)'s names when posting or publishing a photo or video.
